The Safety section of the Daily Operations report is used to capture information on Safety, Inspections, Incidents and Tests.

Safety Data

Enter the safety Report Date and Reporter information. Use this area to record incidents and dates of any safety tests performed during the report interval.

Number of Bunks

The number of bunks is carried over from the Personnel section.

DSLTA

Enter the number of days since last accident.

DSLTI

Enter the number of days since last lost time incident.

JSA

Enter the number of days since the last Job Safety Analysis.

RID Cards

Enter the number of Reportable Incident Description (RID) cards handed in.

No Terminations and Persons Terminated

Enter the number and names of persons terminated (paid off from the rigsite), if applicable.

Job Site Check Performed Today

Activate the check box if a job site check was performed.

Noncomp Received

Activate the check box if a non compliance report was received from any regulatory inspection agency.

Gov Insp

Activate the check box if a government inspection was performed at the site.

Company Insp.

Activate the check box if the site was inspected by a company representative.

Sheen or Spill

Activate the check box if there was a fluid sheen or fluid or chemical spill reported.

Safety Moments

This area allows users to record details for safety moments by Topic, Keywords and a full description in the Comments field.

 

Inspections/Drills

This area allows users to record the dates of last inspections or drills performed on the well site. This area also allows for two custom inspection fields and dates to be defined.

 

Hours/Days Since Inspections

This area allows users to record the hours and day since last inspections performed on the well site equipment. This area allows for up to three custom equipment inspection hours and days to be defined.

 

Gas Concentrations

This area allows users to record the average and maximum allowed concentrations for Connection, Trip, Drill and H2S gases for the well site.

BOP Tests

This area allows users to record details of blowout preventer (BOP) tests performed at the well site.

Connectors Tested

Activate the check box if the BOP connectors were tested.

Pods Tested

Activate the check box if the BOP pods were tested.

 

Pressure Tests

This area allows users to record the date and pressure readings recorded at the well site.

Kick Tests

This area allows users to record Kick Tests on a daily basis.  

 

Incidents

To record an incident at the well site, click the Add Row addrow.gif button to add a new row to the Incidents spreadsheet. Enter the pertinent data regarding the incident in all applicable columns. Activate the Incident Report Sent check box if the Incident was reported.
